claims for you. While filing of insurance claims is a courtesy to our patients, all charges
remain your responsibility from the date services are rendered. We will be happy to
provide you with an estimate prior to treatment, and ask that all co-payments for service
be paid at the time of treatment. Please note that our estimates are subject to error. It is
based on the information provided to us at the time. Therefore, our estimate is an
approximate amount of your personal treatment cost. If there is an overpayment, a refund
will be sent to you. If there is an underpayment, the balance will be billed to you.

If your share after insurance payment is over $1,000 dollars, we are pleased to offer
the following discounts:
- If full payment is made prior to treatment with cash or check, we offer a 5%
discount.
- If full payment is made prior to treatment with credit card, we offer a 3% discount.
- To all senior citizens, we also offer a 5% discount when paying prior to treatment.

- We are please to offer CareCredit financing through the office. Please visit their

- We are pleased to offer Unicorn financing to our patients. This is a convenient, no
initial payment, low monthly payment for dental treatment of $500 to $12,000
